This form is for a single entrant. Family members or sharers of the same plot should each fill out separate entry forms.

Entry forms may be handed in at the Shop on Sundays between 10.30 am and 12.30 pm, in the John Gray Centre on Friday, 8 September between 5 pm and 6 pm or at the Centre on Saturday, 9 September between 8.30 am and 10.30 am.
